HAS YOUR PROGRAM INFORMATION CHANGED?

In order to keep our database current, we request child care providers to inform us of any changes in their child care programs. If you have recently changed your contact phone number, for example, please complete the Child Care Provider Information Form and submit it to us by fax, email, or in person.

As long as we have accurate information of child care programs, we may convey that information to families and community members.
